This policy explains what HabitChoice collects, why it is needed, how it is used, and the choices you have about your information.
1. About this policy
This Privacy Policy explains how HabitChoice collects, uses, stores and shares personal information in connection with the website and health coaching services.
HabitChoice is operated by Tremain Cross, a PREKURE Certified Health Coach in Christchurch, New Zealand.
2. Information HabitChoice may collect
Depending on how you interact with HabitChoice, information collected may include:
- your name and contact details;
- booking and appointment information;
- information you provide in coaching forms, agreements and communications;
- health, wellbeing, lifestyle, goals, habits and other information you choose to discuss during coaching;
- coaching notes, action plans and other records created as part of the coaching relationship;
- programme selection and payment-related records; and
- communications between you and HabitChoice.
HabitChoice seeks to collect information that is reasonably needed to provide and administer its services.
3. Why information is collected and used
Your information may be used to:
- respond to enquiries and arrange Connection Calls;
- provide and personalise health coaching services;
- book, manage and document coaching sessions;
- support agreed goals, actions and accountability;
- communicate with you about your programme;
- process and reconcile payments;
- meet accounting, tax, legal and professional obligations;
- maintain appropriate business and client records; and
- protect the security and integrity of HabitChoice services.
HabitChoice does not use your health information for unrelated marketing without an appropriate basis or permission.
4. How information is collected
Information is generally collected directly from you, including when you contact HabitChoice, complete a form or coaching agreement, book an appointment, make a payment, communicate between sessions, or participate in coaching.
If information needs to be obtained from another person or healthcare provider, HabitChoice will generally discuss this with you and obtain appropriate permission unless collection is otherwise permitted or required by law.
5. Services HabitChoice uses
HabitChoice currently uses a small number of third-party services to operate the business:
Carepatron
Used for practice-related functions such as client records, forms and appointment booking. Information you provide through Carepatron may be processed and stored by Carepatron under its own privacy and security practices.
Stripe
Used to process online payments. Stripe receives the payment and card information needed to process your transaction. HabitChoice does not need to receive your full card details in order to accept a Stripe payment.
Hnry
Used for business accounting and tax administration. Relevant transaction, invoice, client or business information may be processed through Hnry where reasonably required for those purposes.
HabitChoice may also use ordinary telecommunications and internet infrastructure necessary to operate the website, email and phone services. If HabitChoice materially changes the main client-data services it uses, this policy may be updated.
6. When information may be shared
HabitChoice does not sell your personal or health information.
Information may be disclosed where reasonably necessary to provide or administer services, where you have authorised the disclosure, where a service provider processes information on HabitChoice's behalf, or where disclosure is permitted or required by law.
If a serious concern arises about your safety or another person's safety, information may be disclosed where New Zealand privacy law permits or requires that disclosure.
7. Storage and security
HabitChoice takes reasonable steps to protect personal and health information against loss, unauthorised access, misuse, disclosure or alteration. This includes using access controls and reputable service providers appropriate to the nature of the information being handled.
No internet or electronic storage system can be guaranteed to be completely secure. If HabitChoice becomes aware of a privacy breach, it will assess and respond to it in accordance with applicable New Zealand privacy requirements.
8. Retention and disposal
HabitChoice keeps information for as long as it is reasonably needed for the purpose for which it was collected, to maintain appropriate coaching and business records, and to meet applicable legal, tax or professional obligations. When information is no longer required, HabitChoice will take reasonable steps to securely delete, destroy or de-identify it where appropriate.
9. Accessing and correcting your information
You may ask to access personal information HabitChoice holds about you and request correction if you believe it is inaccurate. Some legal exceptions may apply.
To make an access or correction request, email hello@habitchoice.com. HabitChoice may need to verify your identity before releasing information.
10. Email and text communication
Email and text messaging are convenient but are not guaranteed to be completely secure. Please consider this when choosing what information to send through these channels. Between-session communication is intended for brief coaching-related matters and is not an emergency or clinical service.
11. Website and links
The HabitChoice website may contain links to external websites or services. Their privacy practices are controlled by those providers, not HabitChoice. This policy applies to HabitChoice's own handling of information.
12. Privacy concerns or complaints
If you have a privacy question, concern or complaint, please contact HabitChoice first so there is an opportunity to address it.
